To ensure your first visit with us goes smoothly we request the following:
- Bring your insurance card, a form of ID, and a credit or debit card to keep on file for any copays or coinsurance. We are a cashless business.
- If you are interested in having an allergy skin test performed at your first appointment, you will need to stop all antihistamines for 5 days prior to the appointment. This is necessary to get accurate results from the test as these medications mask your body’s response to allergens.
- If you are unable to discontinue taking antihistamines, Dr. Farnam can discuss alternative methods for identifying allergic triggers with you at your appointment.
- Complete the new patient electronic forms prior to your appointment. The electronic forms will be texted and/or emailed to you. Failure to fill out the forms ahead of time could result in reduced time spent with the physician during your appointment.
- We will provide you with a cost estimate for allergy skin testing while you are here for your appointment, but if you would like to find out your insurance coverage prior to the visit you may do so by calling the customer service number located on the back of your insurance card.
- Prepare enough time for the appointment; new patient appointments typically last two hours. There is nearby free street parking with time limits, or you may utilize the on-site parking lot for $8.
